Understanding Brain Fog, Mood Swings, and the Mind-Body Connection in Perimenopause

This post is part of my new series, The Mind of Perimenopause - a gentle, science-meets-soul deep dive of what’s really happening inside your head (and heart) during this time. Together, we’ll unpack brain fog, mood swings, motivation, and mindset and how to bring them all back into balance.

The Real Story Behind Brain Fog

Let’s be honest… brain fog can make you feel like you’re slowly losing your mind - or at least your short-term memory! You walk into a room and forget why you’re there. You reread the same email three times. You start a sentence with full confidence… and then it just evaporates.

My husband laughs (and sometimes sighs) when I jump up the moment he asks me to do something, but honestly, if I don’t do it right then, it’s gone. Completely gone. The struggle is real, and a little bit hilarious when you learn to laugh at it.

But here’s the thing, this isn’t you “getting old” or becoming forgetful. There’s actually a real, biological reason behind it. Estrogen, the same hormone that keeps your cycles regular and your skin glowing, also fuels your brain. It helps your brain cells use glucose, their main energy source.

As estrogen starts to fluctuate (and eventually decline), your brain can’t access fuel as efficiently. It’s like running your laptop on 10% battery... everything slows down. Now add twenty tabs open at once (work, family, hormones, life!) and it’s no wonder the system starts to lag.

Again, nothing’s wrong with you. Your brain is simply adapting to a new rhythm... one that asks for more rest, nourishment, and grace than before.

  • You’re not losing your memory.

  • You’re not “just tired.”

  • You’re running in low-power mode - and your brain is asking for support, not shame.

Over 60% of women in perimenopause report cognitive changes, especially trouble focusing or remembering small things. You’re not alone, and this is 100% reversible once you understand what’s happening.

Brain Fog vs Dementia: What’s the Difference?

If you’ve ever caught yourself thinking, “What if this isn’t just brain fog?” you’re not alone. Many women carry the fear that forgetfulness or losing track of words means something serious.

Here’s the good news: this is not dementia.

Brain fog is a temporary slowdown in how your brain processes information, caused by hormonal changes, stress, blood sugar fluctuations, and even poor sleep.

Dementia, on the other hand, involves permanent structural changes to memory and brain function.

Think of brain fog as a software issue - your system just needs an update and a reboot. Brain fog is inconsistent. Some days you are sharp and other days... not so much.

Dementia is a hardware issue - damage to the physical wiring. Dementia is progressive and steady, and it worsens without relief.

When estrogen dips, your brain literally has less “fuel” to think clearly... so words slip, focus fades, and you feel scattered.

But once you start supporting your hormones, blood sugar, and nervous system, the fog begins to lift.

Your brain isn’t broken - it’s upgrading. You’re simply updating into the next version, one that runs with more wisdom, awareness, and intuition than ever before.

When Hormones Hijack Your Headspace: The Real Link Between Mood and Memory

Think of your brain and hormones as being in the same group chat. When estrogen is balanced, everything runs smoothly: focus, motivation, energy, mood.

But in perimenopause, estrogen doesn’t decline in a straight, predictable line. It’s more like the stock market, predictably unpredictable. One day it’s high, and you feel calm, clear, and productive. Two days later, it crashes, and suddenly you’re emotional, foggy, or anxious for no clear reason.

That’s because estrogen doesn’t just regulate your cycle, it also talks to the brain chemicals that make you feel like you.

  • Dopamine: your motivation, drive, and ability to focus.

  • Serotonin: your calm, emotional balance, and mood stability.

  • Cortisol: your stress thermostat, deciding whether you’re cool and collected or in overdrive.

  • Acetylcholine: your memory and mental clarity messenger. It’s the neurotransmitter that helps your brain store, recall, and connect thoughts with ease.

So in perimenopause, when estrogen swings, it throws those brain messengers off balance too, dopamine, serotonin, cortisol, and even acetylcholine, the one responsible for memory and focus. Because estrogen is the Manager, I mean she runs the entire team.

But she’s going through reverse puberty. One day it’s “Let’s go girls!” full power, focus, and sparkle, the next she’s a barefoot hippie craving naps, snacks, and zero responsibility. Then suddenly… total meltdown mode!

No wonder you’re snappy, panicky, super motivated, or flat for no reason, walking into rooms forgetting why you’re there, or going from ready-to-jump-your-partner one week to please-don’t-breathe-near-me the next.

And remember, she’s still the one running the show! No wonder your brain and body feel like they’re trying to keep up with five different versions of the same boss.

So, it’s not you being emotional, “too much,” or extra (as my boys would say), it’s biology.

Remember, your brain is recalibrating. And once you understand what’s really happening, you can stop fighting against your body and start working with it. With more compassion, nourishment, and grace.

This recalibration doesn’t just affect your mood or focus, it goes deeper. It affects your stress response and even your immune system. That’s where the next layer comes in: emotional inflammation - when your body and emotions start speaking the same language.

Emotional Inflammation

Why some days everything simply feels harder

Some days, it’s not just hormones - it’s inflammation.

Neuroscientists now call this neuroinflammation, which basically means your brain and nervous system are irritated. When that happens, everything feels heavier... your mood, your energy, even your thoughts.

During perimenopause, when progesterone (your keep calm and carry on hormone) drops, cortisol (your stress hormone) tends to rise. This hormone is the one that often drops first at the start of Perimenopause.

Now add poor sleep, less that adequate nutrition, nutrient deficiency, blood sugar spikes, or a stressful conversation or meeting, and you have the perfect storm for:

  • Brain fog

  • Mood swings

  • Morning anxiety

  • That “tired but wired” feeling

It’s not all in your head, it’s in your cells.

Your nervous system and hormones are constantly talking, and when they’re overloaded, your brain sends out a distress signal that you feel as overwhelm, irritation, anxiety, rage, or exhaustion.

But here’s the good news! You can calm this storm. By supporting your nervous system, nourishing your hormones, and giving your body what it actually needs (not punishment or restriction, but rest, food, hydration, sunlight, and grace), you can bring your system back into balance again.

How to Pinpoint a symptom and find a leverage point

Perimenopause isn’t the end of clarity, energy, or focus, it’s a recalibration.

When you understand what’s truly happening inside your body, you stop fighting it. You begin working with it - through better nutrition, mindful rest, nervous system care, and daily grace.

So next time the fog rolls in, remember: your system isn’t shutting down. It’s upgrading. You’re not losing yourself, you’re meeting the next, wiser version of you.
